(And What Most People Get Wrong When They Do)
There’s a shift that happens sometime after 40.
It’s subtle at first. Energy that used to come back after one good night’s sleep now takes two. A word you know but can’t quite reach. Bloodwork that was fine last year comes back with a couple of numbers highlighted — “low normal” for B12, or a homocysteine level your doctor mentions but doesn’t act on. Nothing dramatic. Nothing anyone would call an emergency. Just… different than you used to be.
For a lot of people over 40, part of that shift is B-vitamins.
They’re one of the most under-appreciated nutrients in the aging conversation. Not because the science is obscure — the science is well-established. Just because B-vitamins are unglamorous. There’s no expensive test to sell you, no biohacking hype attached, no viral influencer content. Just a set of small molecules your body needs in slightly larger amounts than it used to, and metabolizes slightly less well than it used to.
Here’s what actually shifts after 40 — and what to do about it.
What genuinely changes after 40
Your B-vitamin requirements don’t rise dramatically at midlife. What changes is your ability to absorb and use what you take in. A few specific things are happening under the hood:
Stomach acid production drops. This is called hypochlorhydria, and it affects roughly 40% of adults over 60. Stomach acid is required to release B12 from the animal proteins you eat, and to prepare it for absorption further down the digestive tract. When acid levels drop, B12 absorption drops with them. This is why B12 deficiency is so common in older adults even when they’re eating adequate amounts of animal foods.
Common medications deplete B-vitamins. Metformin, prescribed to a significant percentage of adults over 45 for blood sugar management, depletes B12 by 10–30%. Proton pump inhibitors (Prilosec, Nexium, Prevacid) reduce B12 absorption by blocking stomach acid. Long-term birth control pills, certain seizure medications, and methotrexate all disrupt the methylation cycle. If you’ve been on any of these for years, your B-vitamin status is probably lower than it should be, and your doctor may not have flagged it.
Chronic stress increases turnover. Stress raises the metabolic rate at which your body burns through B-vitamins — especially B5, B6, and folate. Chronic, low-grade stress (the kind most adults over 40 are living with) means chronic, low-grade B-vitamin depletion.
Sleep quality declines. Fragmented sleep raises the body’s metabolic demand for B-vitamins. If you’re not sleeping the way you did at 25, your body is asking for more of these nutrients than it used to.
Genetic variants become more consequential. MTHFR, COMT, and MTRR variants that were mostly invisible in your 30s can start showing up as clear symptoms in your 50s. Not because the variants change, but because the cumulative effect of slower methylation adds up over decades.
None of this means anything is broken. It means the terrain your body operates in has shifted, and the supports you need have shifted with it.
What B-vitamins actually do (and why it matters after 40)
B-vitamins do a lot, but a few functions are particularly relevant at this stage of life:
Cognitive function. B12 deficiency has been shown in the medical literature to mimic early-stage Alzheimer’s disease. There’s a documented pattern of older adults being diagnosed with dementia when the actual root cause was undetected B12 deficiency. This isn’t rare. It’s just not well-known outside of specific medical specialties.
Cardiovascular health. B-vitamins (especially B12, folate, and B6) regulate homocysteine — an amino acid that, when elevated, is a validated cardiovascular risk marker. Elevated homocysteine is associated with increased risk of heart disease, stroke, and cognitive decline.
Energy and mitochondrial function. B-vitamins are cofactors in the biochemical reactions that produce cellular energy. When they’re low, energy production falters. That “tired all the time” feeling so many people over 40 report isn’t always about sleep — sometimes it’s about the fuel-processing machinery running on empty.
Mood. Deficiencies in B12, folate, and B6 are associated with increased depression risk. In people who don’t respond to standard antidepressants, correcting an underlying B-vitamin deficiency has been shown in some cases to change the picture significantly.
Bone health. Elevated homocysteine has been linked to reduced bone mineral density and increased fracture risk. The mechanism is partly related to how homocysteine interferes with collagen cross-linking, but the association is real — and it’s another reason B-vitamins matter more as you age.
Nerve health. B12 deficiency causes peripheral neuropathy — the tingling, numbness, or burning in hands and feet that many older adults dismiss as “just aging.” It’s often reversible if caught early. If ignored, the nerve damage can become permanent.
Signs you might be low
If you’re over 40 and any of the following ring true, you may be low in one or more B-vitamins:
- Fatigue that doesn’t respond to sleep
- Brain fog or noticeable memory changes
- Tingling, numbness, or burning in hands or feet
- Persistent low mood or worsening depression
- Elevated homocysteine on recent bloodwork
- Cracks at the corners of the mouth (a folate/B2 sign)
- Depression that hasn’t responded to other approaches
- Difficulty concentrating or losing your train of thought mid-sentence
These aren’t diagnostic on their own — many other things can cause similar symptoms. But they’re worth paying attention to. If several show up together, ask your doctor for a B12 test, a serum folate test, and a homocysteine panel.
The catch: not every B-complex is a good B-complex
Here’s where a lot of people over 40 go wrong.
They decide (or their doctor decides) that a B-complex would be a good idea. So they walk into a health food store, or scroll through Amazon, and buy whatever looks “premium.” The label promises high potency, active forms, methylated bioavailability, mega-doses — all the marketing language that suggests “more is better.”
For B-vitamins specifically, more is often not better. And the wrong choices can actually create problems:
B6 over 100 mg/day has documented neurotoxicity risk. This means nerve damage — usually starting in the hands and feet, ironically the same symptoms B-vitamin deficiency causes. This is well-established in medical literature. Many “premium” B-complexes contain 25–100 mg of B6 per serving. Over years of daily use, real risk accumulates.
B12 above 1,000 mcg is largely wasted. Your body’s absorption drops sharply at high doses. At 5,000 mcg of B12, you might absorb 15 mcg. The rest turns your urine bright yellow. You paid for the whole dose. Your body used a fraction of it.
High-dose biotin (5,000+ mcg) interferes with common lab tests. Including thyroid panels and troponin tests (used to diagnose heart attacks). The FDA has issued public safety warnings about this. Your bloodwork can come back inaccurate if you’re on a high-dose biotin supplement.
Methylated forms aren’t “superior.” They’re just faster. If your body already converts non-methylated forms fine — which is true for most people, even most MTHFR carriers — methylated forms are unnecessary. In roughly 1 in 3 adults with slow methylation downstream (through COMT, MAOA, or general methyl sensitivity), methylated forms actively cause overstimulation, anxiety, and sleep issues. There’s no reliable way to tell in advance which group you’re in without genetic testing or a careful trial.
Fillers matter. Artificial colors, magnesium stearate, titanium dioxide, and other additives serve no nutritional purpose. Some can cause reactions in sensitive people. The shorter and cleaner the “other ingredients” list, the better.
What to look for on any B-complex label

If you’re going to take one — and after 40, it’s often a good idea — here’s what to look for:
- Non-methylated forms of B12 (cyanocobalamin or hydroxocobalamin), folate (folic acid), and B6 (pyridoxine HCl). Unless you have a specific reason to need methylated forms, non-methylated is safer, more predictable, and works for the widest range of people.
- Moderate doses. 100–500% DV per nutrient is plenty. Anything over that isn’t giving you more benefit — it’s giving you diminishing returns AND accumulating risk, especially with B6.
- No unnecessary “methylation stack” ingredients. TMG, betaine, DMG, and SAM-e are often added to B-complexes “for support.” They can compound methyl overload in sensitive people and aren’t necessary for most.
- Clean ingredient list. No artificial colors, minimal fillers, nothing in the “other ingredients” you can’t recognize.
- Liquid format is a plus. It lets you adjust drop-by-drop instead of being stuck with whatever’s in a capsule — useful if your body is sensitive or if you’re just starting out.

After 40, B-vitamins go from “nice to have” to “worth being intentional about.” The absorption issues, the medication interactions, the accumulated impact of genetic variants — all of it adds up. Supplementation, done right, can meaningfully change how you feel.
Just don’t grab whatever’s on the shelf.
The right B-complex — moderate doses, clean forms, no unnecessary additives — can support your energy, cognition, mood, and cardiovascular health for decades. The wrong one can create new problems that are worse than the deficiency you were trying to address.
Be intentional. Read the label. Use the reference card. Choose something built for careful long-term daily use, not for looking impressive on the shelf.
